A. UNDERGRADUATE APPLICATIONS

A1. Course Recognition Application[JUSTIFICATIONS]

  • Copy of detailed course grades to be recognized by the Department of Origin

  • Description of the course or courses of the Department of Origin based on which recognition is requested (see Department of Origin Study Guide)

  • The form should clearly indicate the course of our Department (with its code and title, see study guide) with which course of the Department of origin the student requests to be recognized.
    After submitting his request tohttps://eservice.upatras.gr, the student should send the resulting pdf form to the email address secr-math@math.upatras.gr with the subject: "Application for recognition of courses to the student's name and surname" and attach the supporting documents.

 A.2Application for Suspension of Study for Undergraduate Students[JUSTIFICATIONS]

According to article 76 of Law 4957/2022 entitled: "Maximum duration of study and part-time study" and specifically in paragraphs 4 and 5 concerning the procedure for granting study suspensions, the following is stated:

"...4. Students who have not exceeded the maximum number of years of study referred to in paragraph 1 may, upon application to the Department Secretariat, interrupt their studies for a period not exceeding two (2) years. The right to interrupt studies may be exercised once or in parts for a period of at least one (1) academic semester, but the duration of the interruption may not exceed a total of two (2) years if granted in parts. Student status is suspended at the time of interruption of studies and participation in any educational process is not permitted. According to the internal regulations
of operation of the AEI, the procedure for determining the interruption of studies and the supporting documents that accompany the application are determined.

  • Any outstanding issues with the Student Residences, the University of Patras Library, catering and Erasmus will be checked by the competent services.

  • After submitting his request tohttps://eservice.upatras.gr, the student should send the resulting pdf form to the email address secr-math@math.upatras.gr with the subject: "Application for Suspension of Studies to the student's name and full name" and attach the supporting documents.

 A.3Application for Certificate of Completion/Completion of Studies for Undergraduate StudentsIn case the application is submittedBEFOREthe Graduation Datefrom the Department.[JUSTIFICATIONS]

  • Copy of both sides of your ID card.

  • Copy of academic ID.

  • Responsible Declaration of Loss or Non-Issue of Academic Identity from https://www.gov.gr, in case the student has lost or has not issued an Academic Identity.

  • Other required documents:Oath application form provided by the Secretariat.

  • After submitting his request tohttps://eservice.upatras.gr, the student should send the resulting pdf form to the email address secr-math@math.upatras.gr and the other supporting documents in ONE message containing the files separately (1 file orapplication, 1 file orsolemn declaration, 1 file or the identity card with both sides), to the email address: secr-math@math.upatras.gr with the title: Application for Completion of Studies on the A.M. and the student's full name. The title of the email as well as the completion of the information on the forms should be in small letters and in bold!

In case the application is submittedAFTERthe Graduation Dateby the Department.[JUSTIFICATIONS]

  • Copy of police ID.

  • Presentation of the original Academic ID to the Department Secretariat (NECESSARY TO COMPLETE THE PROCEDURE).

  • Responsible Declaration of Loss or Non-Issue of Academic Identity from https://www.gov.gr, in case the student has lost or has not issued an Academic Identity.

  • Other required documents:Oath application form provided by the Secretariat.

 A.4Application for Declaration of Completion of a Bachelor's Thesis  [JUSTIFICATIONS]

  • Formapplicationgranted by the Secretariat

  • After submitting his request tohttps://eservice.upatras.gr, the student should send the resulting pdf form together with the relevant application to the email address secr-math@math.upatras.gr with the title: Application for the preparation of a Bachelor's Thesis on the A.M. and the student's full name. The title of the email as well as the completion of the form details should be in small letters and bold!

 A.5Deletion Request [JUSTIFICATIONS]

  • Copy of police ID.

  • Presentation of the original Academic Identity Card to the Department Secretariat (NECESSARY TO COMPLETE THE PROCEDURE).

  • Responsible Declaration of Loss or Non-Issue of Academic Identity from https://www.gov.gr, in case the student has lost or has not issued an Academic Identity.

  • Any outstanding issues with Student Housing, the University of Patras Library, catering and Erasmus will be checked by the competent services.

  • After submitting his request tohttps://eservice.upatras.gr, the student should send the resulting pdf form to the email address secr-math@math.upatras.gr along with the other supporting documents in ONE message that will contain the files separately (1 file orapplication, 1 file orsolemn declaration, 1 file the ID card with both sides, 1 file the academic ID card), with the title: Request for Deletion of the A.M. and the student's full name. The title of the email as well as the completion of the form details should be in small letters and bold!

 A.6Late Course Registration Request [JUSTIFICATIONS] Not required, only if they indicate the reasons why the declaration was not made on time

In the electronic application, the course, the course code, and the semester in which the student wishes to register for each course are filled in. 

A.7Application for Reconsideration of Procedural Grade [[JUSTIFICATIONS] Not Required

Students have the right to register for re-examination ONLY the CORE courses of their current academic year.The request should state:AND the teacherthat the student had chosen during the semesterIn case of non-compliance with the directive, the student's application will not be processed by the Secretariat. 

A.8Application for Undergraduate Student Oath TakingIn case a previous Application for Completion Certificate has been submitted[DOCUMENTATION] No Supporting Documents are required to be attached to the Oath Application again.In case a previous Application for Completion Certificate has NOT been submitted [JUSTIFICATIONS]

  • Copy of police ID.

  • Presentation of the original Academic Identity Card to the Department Secretariat (NECESSARY TO COMPLETE THE PROCEDURE).

  • Responsible Declaration of Loss or Non-Issue of Academic Identity from https://www.gov.gr, in case the student has lost or has not issued an Academic Identity.

  • Other required documents:Oath application form provided by the Secretariat.

  • Any outstanding matters with the Student Residences, the Library of the University of Patras, catering and Erasmus, will be checked by the competent services

 A.9Application for Participation in the Qualifying Exams[JUSTIFICATIONS]

  • Copy of police ID.

  • Copies of Degrees.

  • Certificates of Analytical Grades.

  • Decision on the Equivalence of Academic Qualifications (where required).

No Supporting Documents Need to Be Attached Again to the Oath Application

 

In the event that a previous Application for Approval for the Award of a Postgraduate Degree has NOT been submitted

  1. Copy of police ID.

  2. Presentation of the original Academic Identity Card to the Department Secretariat (NECESSARY TO COMPLETE THE PROCEDURE).
